You have a right to have your complaints heard and resolved in a timely manner. If we cannot work things out to your satisfaction, you may file a complaint with my licensing board:

How to Request Your Health Care Records:

As a consumer, you have the right to access your health care records. You may request your health care records by submitting a written request by mail. Please include your full name, date of birth, contact information, and specify the records you are requesting.

For your privacy and security, we may ask you to verify your identity before processing your request. Once your request is received, we will respond within 15 days as required by Texas law.
